GRADE: 4-8 / TIME: About 1 Week + Reflection Block ~ REUSABLE

Healthy Habits Diary & Reflective Activity (5 Pages)

Help students develop self-awareness and make meaningful connections between their daily habits and overall well-being. Designed for elementary and middle-grade classrooms. This resource guides students to track their sleep, food and drink choices, physical activity, and emotions throughout the day. Using the completed tracker, teachers can facilitate insightful class discussions about patterns students notice; such as how food, hydration, movement, and rest affect mood and energy levels.

NOTE ~ Part 1, is kept at school and Part 2 is brought home for students to record their evening activities.

Reflection Activity Idea: Students stand across from one another, along a line on the floor, and step forward in response to "yes" answers (e.g., “Did you sleep better on days when you drank more water?”). This visual and physical strategy promotes empathy, self-reflection, and awareness of healthy routines of self and peers, all while building classroom community.
